Duplicate annotations still showing up #16

Exploring it further:

If I do a refresh of the page, the barrat impulsiveness scale instrument has a single annotation. If I then navigate away, to Study, and navigate from a specific study to the related instrument, barrat impulsiveness scale, then it has duplicate annotations:

A different example shows two annotations with the same tag, but with different identifiers (not sure if for the same Agent or different ones...)
